Assumption Of Risk
Combat sports and physical training — including but not limited to boxing, Muay Thai, kickboxing, Brazilian Jiu Jitsu, strength & conditioning and related activities — carry an inherent risk of injury, including serious injury. I acknowledge that I am voluntarily participating in classes and activities at Relentless Striking Academy ("the Academy") and I freely accept and assume all risks, known and unknown, associated with my participation.
Medical Fitness
I confirm that I am medically fit to participate in physical exercise and combat sports training. I have disclosed any relevant medical conditions, injuries or limitations above. I understand it is my responsibility to inform coaching staff of any change to my health or fitness to train, and to consult a medical professional before beginning any new exercise program if I am unsure of my fitness to do so.
Release Of Liability
To the fullest extent permitted by law, I release, discharge and hold harmless Relentless Striking Academy, its owners, coaches, staff, and Ultra Renovations Group (facility fit-out partner), from any and all claims, demands, actions, or liabilities arising from personal injury, illness, death, or property damage that may occur while participating in classes, using the facility, or as a result of my own or another participant's actions, except where such injury or damage is caused by the gross negligence or wilful misconduct of the Academy.
Photo & Video Release
I consent to the Academy photographing or filming me during classes and events for use in promotional material, social media and the website, unless I notify the Academy in writing that I do not consent.
Membership Terms
I understand that memberships are based on a minimum 3-month commitment, billed by Direct Debit at the frequency I have selected above, and that a one-off Registration Fee applies on joining. I understand exact pricing will be confirmed with me by Academy staff prior to my first payment, and no payment is processed as a result of submitting this form.
Direct Debit Request Authority
I authorise Relentless Striking Academy to arrange, through its nominated Direct Debit provider (Ezidebit), for funds to be debited from the bank account I have provided above, in accordance with the payment frequency I have selected and the membership fees confirmed with me by Academy staff. I understand this authority remains in place until I cancel it in writing, and that I should refer to the Direct Debit Request Service Agreement provided by Ezidebit for full terms, including notice periods for changes or cancellations. I confirm the bank account details provided are accurate and that I am authorised to operate this account.
Code Of Conduct
I agree to train respectfully, follow coach instructions, and treat all members and staff with courtesy. I understand the Academy reserves the right to suspend or terminate membership for conduct that endangers others or breaches this code.
